Pulling all Psychiatry/Therapy notes that have been completed within the last 24hours

Daily Workflow - Critical Errors

  1. Each day, session notes are pulled in from the past 24hours and are audited in Brellium, to then show up in the audit results.

  2. Audits with Critical Error’s will trigger an email that gets sent to the Manager, regarding that Provider’s documentation issue

  3. Managers are responsible for enforcing any amendments or billing adjustments needed, based on the email that has been received

  4. If the Manager or Clinician disagrees with the ‘Critical’ error identified, they can log into Brellium and share feedback using the comment box to the right of each question to let us know if the question’s logic to be changed.

Monthly Workflow - Clinical Criteria

  1. Each month, providers session notes are aggregated into an overall quality score, which then tells Brellium which feedback email that they will receive:

    1. Providers who score >90% receive an email with 3 examples of their top criteria and one ‘area for improvement’

    2. Providers who score >60% and <89% receive an email with 3 examples of their top criteria and 3 ‘areas for improvement

    3. Providers who score <60% receive an email with 3 ‘areas for improvement’ and some suggested resources to assist with documentation quality

2. Managers are cc’ed on the emails and are responsible for helping clinicians with areas of improvement and providing resources as needed

Monthly Workflow - Overall Trends

New Providers:

If Managers have new providers, it may make sense to go through their audits for the week. Even if they have minor errors, it is probably best to catch those early so they keep doing well and so that they know someone is looking at their notes. You can filter by “Provider” and then select any new providers that you’d like to review.

Important Questions:

If there are any question that Managers or the QA team are particularly interested in that are not marked as Critical, they can go through all audits that fail these questions. You can create a flag and apply it to a question or group of questions. For example “Safety Plan” would allow you to filter on any notes where Therapists might need training regarding timeliness of creating and reviewing safety plans. This will ensure that no audits slip through the cracks that are failing important questions or groups of questions.

Trend Data:

It can be helpful for Managers to keep track of providers that are continually failing or excelling on certain criteria. You can use Trend Data to get an accurate view of people’s average scores and their scores per question. If you find that a particular provider is not improving week over week, or they are struggling on critical questions you could set up some time with them to discuss.

Monthly Workflow - Upkeep

  • We recommend Clinical team members continually assess the questions in Brellium to ensure that we are staying at a high level of agreement and are up to date with changes in the note templates or requirements at your organization.

  • To accomplish this, Clinical teams will often review a representative sample of audits for each month and ensure that they are in agreement with the question responses and the context provided. Typically a strong benchmark level of agreement between one clinical member and Brellium is 80%, although this depends on the level of subjectivity in the questions and other factors.

  • If you find that there are specific areas of questions that need to be adjusted, please leave feedback on the questions by clicking the comment box to the right of a question, within the audit itself, or by requesting a change on the far right of a question in the question studio.
